The review sheet.

This is the longer sheet behind a Preliminary Risk Review. It asks the questions a Florida carrier will ask anyway — roof age, plumbing and panel, claims, flood — so the first conversation starts from your actual exposures instead of a questionnaire.

If you would rather simply say hello first, the short form is there. Sending this does not bind coverage, and we never ask for a Social Security number, a driver license number or a credit-consent answer.

Fields marked with an asterisk are required. Everything else is optional — tell us what you know and we will ask about the rest.

“I don’t know” is a real answer

Several questions below offer it, and they mean it. A guess about your plumbing or your roof year can follow you onto an application and cost you a claim years later. If you are not sure, say so and we will find out properly.

Why we ask about losses plainly

We ask you to state whether there have been claims. A blank is not a “no”. A file that says the wrong thing about losses is the most common way a claim later gets denied.
